More long-lasting depression is often missed and the degree of disorder may be hard to assess. The family planning doctor, whether in general practice or in a clinic, needs to look carefully for signs of clinical depression as the patient may not complain spontaneously. The routine visit for a repeat prescription for contraception is an important opportunistic moment for screening for such illness.<br />

Although she had married on the understanding that neither of them wanted to have children, underneath she had been sure that if she changed her mind it would be easy enough to persuade her husband to think differently as well. As the years passed she began to realize that she did want a baby but found she was unable to sway her husband. She was angry that he persisted in his rebellion against the family life that she now desired; angry that he had been made redundant from his job; and angry that he should expect to have sex with her when it was so painful.<br />

Slowly, as the consultation continued and with the doctor trying to respect the girl&#8217;s feelings and treat her as a responsible person in her own right, the girl changed, her face softened and she allowed herself to look her age. She said that she did not wish to decide anything that day, and certainly did not want to take any pills.<br />

The title of this book, Contraceptive Care: Meeting Individual Needs, draws attention to the contrast between the needs and perceptions of individual health professionals and the individual needs of those they are trying to help. It could be argued that the word &#8216;care&#8217; is hardly relevant here, certainly if used in its literal sense of watching over or tending (Hawkins, 1989). Might we be tempted to use this knowledge, this care, to intrude into, even try to control, other people&#8217;s private sexuality? And yet, modern contraception is complicated, often highly technical and scientific, and our patients do feel the need to seek our advice. Clearly, doctor and patient share the same difficulty when it comes to balancing emotional and physical needs in this field. Patients bring to even the most apparently uncomplicated consultation their own hidden agenda, which doctors and nurses, intent on the rational and scientific aspects of birth control, frequently miss.<br />

There have been no long-term studies of how well they work, and how they compare to surgical treatments for BPH. When a patient with BPH really needs treatment, surgery is still the option most urologists prefer, for several reasons. One is that pills must be taken daily; their effect ends soon after you stop taking them, and a prescription might well mean taking these pills for life. Also, it may take weeks on medication before any improvement is noticeable. The TUR procedure, particularly, has faster and more effective results than any medications for BPH currently available. Nonetheless, because surgery is not an option for all men—and because many men have symptoms that are annoying but not severe enough to warrant surgery—the availability of drugs means more treatment alternatives than ever before. And perhaps just knowing there&#8217;s something out there besides surgery will encourage some men who until now have been reluctant to seek help for their symptoms.<br />

<p><span style="font-family:Courier New; font-size:10pt">There are two tiny bundles of nerves, one on either side of the prostate, that are essential for erection. Sometimes, depending on the extent to which the cancer has spread, one or both of these bundles must be removed. Men can still be potent even if one — but not both—of these bundles is removed. In men over so, sexual potency is better in men who have both of these bundles preserved than in men who lose one bundle during surgery.<br />
